Unfortunately I dont. I had the same issue. Apparently after u get set up woth a doctor is when you put in your request. I havent gone that far with it because i didnt have to, yet.

@kopf134 Thank you so much for your reply. It appears this app connects you with doctors but you can’t put a request in. Do you know how to get a consultation with a doctor who would write an exemption?

Delta HR opened a window by allowing us to file for an “ethical” waiver and I was encouraged by the CPO to do so. I did and after 2 months they finally ruled on it and DENIED my ethical waiver!!… Read more»
